It is our policy to inform you of the estimated expenses for treatment and assist you in determining your insurance benefits. Please do not hesitate to ask for clarification at any time.  We want to make every effort to assist you in getting the care that you need.

What is HUSKY?

The HUSKY Plan is a subsidized health insurance program to help Connecticut families obtain and afford coverage for their children. By becoming a HUSKY dental provider, we are providing services and assisting local children ages 6-20, who really need our help.
 
 

Our staff will file claims for you, asking only for your co-payment or the amount not covered by your carrier. It is always a good idea to contact your employer or insurance advisor to obtain precise information about the extent of your coverage prior to treatment. At East Lyme Oral & Maxillofacial Surgery, LLC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date.

For your convenience, we process many major insurances in addition to accepting cash, check, money orders, Visa, MasterCard, Discover, American Express, and CareCredit.  We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
