First Visit
Once your consultation appointment has been made, please complete our secure online New Patient Forms.  This form needs to be completed before your scheduled appointment time.  Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:
  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.  Please bring both your medical and dental insurance cards with you to your appointment.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A preoperative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, asthma, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, bone density medications, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they send them to our office. If there is not enough time, please pick them up and bring them to our office. We can download emailed xray images, too. If additional films are necessary, they can be taken at our facility.
 

Online Patient Registration

You may preregister with our office by filling out our secure online New Patient Forms.  After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.  We are now able to remind patients of appointments via text messaging, emails and answering machines.

Technical Note:

PC Users

Our online form uses Adobe Acrobat Reader 9 or greater plug-in to conveniently submit the form from home or work. Please download the free plug-in from Adobe's web site if it is not already installed on your system. It is important that you have at least version 9 of the plug-in to successfully use our online form.

Mac Users

You must open and submit the form in a Safari Browser with the latest Mac operating system. It is also important to have the latest version of Adobe Acrobat Reader on your computer in order to submit your form to our office correctly, please download the free plug-in from Adobe's web site.
