The HUSKY Plan is a subsidized health insurance program to help Connecticut families obtain and afford coverage for their children. By becoming a HUSKY dental provider, we are providing services and assisting local children ages 6-20, who really need our help.
Our staff will file claims for you, asking only for your co-payment or the amount not covered by your carrier. It is always a good idea to contact your employer or insurance advisor to obtain precise information about the extent of your coverage prior to treatment. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
